Application Instructions
To be considered for admission to the online Master of Science in Health Informatics program, you must submit all of the following items. Please follow each step in the application process carefully, as errors may delay the processing of your application. If you have any questions, please do not hesitate to send an email, or call (866) 772-2268, option 1.
Once your application materials are received and reviewed, your admissions advisor will notify you of any additional information that is required.
Grad School Application
Visit the UIC Graduate Application page. Once on the page, you can either search for the program or select the Master’s Programs link under the program search bar. Select, or search for, Health Informatics – MS from the list of available programs. After you have selected Health Informatics – MS, click on Apply and create a log in.
You will be able to complete Steps 2 – 7 (listed below) within the online application portal.
Complete and submit the application along with the $70 non-refundable application fee.
Official Transcripts
You will upload official (electronic) transcripts within the online graduate application, for each institution you have attended.
International Application Fees
If you are an international applicant or have taken foreign coursework, please refer to the foreign credential guidelines on the International Admission Requirements webpage. An international credential evaluation fee of $100 is assessed to any applicant presenting coursework taken outside the United States. The evaluation fee must be paid after submission of the application.
Letters of Rec
Fill out the form, within the application portal, to have the graduate college send out email requests for 2 recommender surveys. You will need to input the recommenders email addresses.
Suggested questions to forward to your recommenders, which they can address in their letters of recommendation:
- What is the student’s motivation for graduate study?
- What is the student’s conceptual/analytical ability?
- What is the student’s initiative and potential for research?
- How would you describe the student’s integrity?
- What is the student’s ability to work with others?
- What is the student’s effectiveness in speaking and writing?
Resume
Be sure to upload your most recent Resume, or CV, that details educational background, work experience, and honors/awards/activities, if applicable.
Personal Statement
Upload a personal statement that describes to the admissions staff who you are and why you want to attend the program. Be sure to include life experiences; why you want to pursue the degree; any particular fields of study that interest you; professors you are interested in having mentor your studies; goals that you want to achieve through the program.
International Student Requirements

TOEFL scores must be submitted directly from the Educational Testing Service to the UIC Office of Academic & Enrollment Services using institutional code 1851.
IELTS scores must be submitted directly from the International English Language Testing System to the UIC Office of Academic & Enrollment Services using institutional code 001851.
English Proficiency Requirement Exemptions
The English proficiency test requirement may be waived under the following circumstances, if the conditions have occurred within the last five years:
- Enrollment at a U.S. institution for at least two years of full-time study.
- Exemptions based upon enrollment will be applied after receipt of applicable academic credentials.
- Employment in the U.S. for at least one year on a full-time basis.
- Documentation for exemptions based upon employment will be requested to be uploaded through the application checklist after submission of the application.
- It should consist of a letter from your employer issued on letterhead which indicates the length of time you have been employed at the establishment and attests to your proficiency with the English language.
- Education or employment for the duration of time listed above in a country or territory from which an English proficiency test is not required.
FAFSA
After completing the UIC Graduate Application, be sure to fill out the FAFSA if you are applying for financial aid. Use code number 001776 to designate the University of Illinois at Chicago. http://www.fafsa.ed.gov
