To be considered for admission to the BS in Health Information Management degree, you must submit all of the following items. Please follow each step in the application process carefully, as errors may delay the processing of your application. The BS in Health Information Management program requires all pre-requisites course requirements be completed to qualify for admission. Gather Up Materials
Gather copies of your unofficial transcripts or degree audit for all institutions attended, visit the BSHIM transfer page, download the BSHIM transfer guide, and follow the step-by-step instructions to complete an evaluation of your previous credits using Transferology. Next contact us to review your evaluation results and determine your eligibility to start the program.

Once your application materials are received and reviewed, your admissions advisor will notify you of any additional information that is required.

Undergraduate School Application
Transfer applicants must complete either the UIC Web Application or the Common Application on the UIC Admissions website online application. The major code for the online Health Information Management program is 2PGF1304BSU.
The cost to apply is a $60 nonrefundable application fee. Students with international transcripts will need to pay an additional $50 evaluation fee.

Official Transcripts
An official transcript mailed directly from each college or university you have attended must be submitted. Transcripts marked ‘issued to student’ are considered official only if they are received in a sealed envelope from that school.

International Application Fees
If you are an international applicant or have taken foreign coursework, please refer to the foreign credential guidelines on the International Admission Requirements webpage. An international credential evaluation fee of $100 is assessed to any applicant presenting coursework taken outside the United States. The evaluation fee must be paid after submission of the application.

Letters of Rec
Three completed recommendation forms are required and may be obtained from academic advisors, college professors, employers or others familiar with your past academic achievements or work history. Letters from friends or family members are not accepted.
- Applicants will be asked to identify their recommenders, including a contact email address, through the online application system.
- The system will automatically generate an email to each of the recommenders requesting that recommendations be uploaded to the system. Recommenders should upload a completed copy of the Recommendation Form to the system.

Resume
Be sure to upload your most recent Resume, or CV, that details educational background, work experience, and honors/awards/activities, if applicable.

Course Outline Form
Next to each course indicate where the course was completed with the term and year and the grade received, as well as courses in progress. This is located within the online application as a supplemental upload.

TOEFL scores must be submitted directly from the Educational Testing Service to the UIC Office of Academic & Enrollment Services using institutional code 1851.
IELTS scores must be submitted directly from the International English Language Testing System to the UIC Office of Academic & Enrollment Services using institutional code 001851.
English Proficiency Requirement Exemptions
The English proficiency test requirement may be waived under the following circumstances, if the conditions have occurred within the last five years:
- Enrollment at a U.S. institution for at least two years of full-time study.
- Exemptions based upon enrollment will be applied after receipt of applicable academic credentials.
- Employment in the U.S. for at least one year on a full-time basis.
- Documentation for exemptions based upon employment will be requested to be uploaded through the application checklist after submission of the application.
- It should consist of a letter from your employer issued on letterhead which indicates the length of time you have been employed at the establishment and attests to your proficiency with the English language.
- Education or employment for the duration of time listed above in a country or territory from which an English proficiency test is not required.
- Enrollment at a U.S. institution for at least two years of full-time study.
